We take the time to ensure all your paperwork is properly filled out and insurance, if any, is entered correctly. Dr. Attaway strives to make your visit easy and stress-free when meeting our dental professionals for the first time.

Download and Print New Patient Registration

After you schedule your appointment, we encourage you to download and print our new patient forms, so you can fill them out at your convenience. You can either fax or mail to us or bring them with you to your first appointment.
